These educational examples explore core physiology. They are not official AACN exam questions, recalled exam items or patient-specific clinical advice.

## 1. Oxygen saturation and oxygen content

Two hypothetical adults have the same arterial oxygen saturation, arterial oxygen partial pressure and cardiac output. One has a lower hemoglobin concentration. Which statement best describes the effect of lower hemoglobin?

- Arterial oxygen content is lower, so oxygen delivery is lower.

- Arterial oxygen content must be identical because saturation is identical.

- Cardiac output must be higher because hemoglobin is lower.

- Oxygen delivery is determined only by arterial oxygen partial pressure.

Show answer and rationaleAnswer: A. Hemoglobin carries most of the oxygen in arterial blood. At the same saturation, less hemoglobin means less hemoglobin-bound oxygen per volume of blood. Oxygen delivery depends on both arterial oxygen content and cardiac output, so it falls when content falls and output stays constant.

Reasoning checkpoint: Saturation describes the proportion of hemoglobin binding sites occupied by oxygen. It does not tell you how much hemoglobin is available. Do not assume that a reassuring saturation proves adequate oxygen delivery.

## 2. Recognizing the primary acid-base process

An arterial blood gas shows pH 7.28, PaCO₂ 60 mmHg and bicarbonate 27 mmol/L. Which primary process best explains the acidemia?

- Respiratory alkalosis

- Respiratory acidosis

- Metabolic alkalosis

- A primary fall in bicarbonate

Show answer and rationaleAnswer: B. The pH indicates acidemia. The raised carbon dioxide moves pH in the acid direction, supporting a primary respiratory acidosis. The bicarbonate is not reduced, so a primary bicarbonate loss does not explain these values.

Reasoning checkpoint: Identifying the primary process is only a first step. Time course, expected compensation and the clinical picture are needed to evaluate additional processes and causes. These numbers alone do not establish a treatment or ventilator setting.

## 3. Connecting flow to oxygen delivery

In a simplified physiology comparison, cardiac output falls by half while arterial oxygen content remains unchanged. What happens to systemic oxygen delivery?

- It stays the same because the oxygen content is unchanged.

- It doubles.

- It falls by half.

- It can only change if oxygen saturation changes.

Show answer and rationaleAnswer: C. Oxygen delivery is cardiac output multiplied by arterial oxygen content, with consistent units. Holding content constant makes delivery proportional to flow. Halving flow therefore halves delivery.

Reasoning checkpoint: This simplified relationship describes delivery, not whether a particular patient is meeting metabolic demand. Consumption, extraction and the wider clinical context still matter.

## Turn a missed question into a study target

Before repeating a question, explain what you missed: a fact, a relationship between findings, or the wording of the question. Write one sentence explaining the correct mechanism and another explaining why your chosen alternative fails. Revisit that topic in your study material, then test the idea in a different scenario.

For example, missing the first question is a reason to revisit oxygen content and delivery together, rather than memorizing a single saturation threshold. Missing the second is a reason to practice a consistent acid-base sequence before adding more complicated mixed disorders.
